Usually the ACTIV8 session: what you're trying to achieve, how the work gets done, where it's stuck, what AI is already doing. The plan comes out of that.

Not a predefined list of agents. What does this business actually need, and what's the smallest build that gets it? You get a proposal that says exactly that.
